How to order:

• Only healthcare provider or physician licensed in their state may order genetic tests.

• If you have not previously set up an account, ordered testing/assay from us, please complete the New Account Registration Form and fax it to (310) 794-5652  prior to sending specimens and test requests.

• If contracting for Clinical Trials or special studies please Contact Us for specific instructions.
